OFA Clinic
All OFA exams must be completed last. If your dogs are receiving other services along with OFAs, all other services must be preformed first. Below is a link to the OFA forms that will need to be filled out prior to taking the radiographs, please save time by printing these forms and having them ready when you arrive.
